Tomohiko visits the hospital in "Rough Sketch" while Kōichi Sakakibara was recovering, but he seemed to act very strangely around him, along with his fellow class rep, Yukari, and the countermeasures head, Izumi Akazawa. He was among the students who officially welcomed Kōichi on his first day of school, but because he needed permission from Izumi, he and Naoya didn't tell him about the curse.
Tomohiko was more often than not a background character from then on out. In "Bone Work", when the exam schedule is posted, Tomohiko, along with Naoya, has the most conversation about it and what high school each is likely to attend. He and Naoya live next door and have known each other since they were little. Later in the episode, he and Kōichi are walking between buildings when Kōichi looks up, sees Mei on the roof, says "Hang on," and runs off. Tomohiko is soon seen with Naoya as Naoya calls Kōichi to tell him that Izumi is furious at him for "messing with things that don't exist".
When Izumi declared the class needed to ignore Kōichi in addition to Mei, Tomohiko was one of the students who did so. Following this development and the death of Yukari, he became more closely involved with countermeasures and was occasionally seen accompanying Izumi and Takako Sugiura on breaks. Tomohiko attended the class trip to the shrine in Episode 10, where he shared a room with Naoya.
When Daisuke Wakui had his asthma attack, Tomohiko, sitting to Daisuke's right, was one of the students who tried to assist him. Shaken by this, he and the other students returned to their rooms. In Episode 11, Naoya burst into Mei's room and revealed he'd thought Tomohiko was the dead person, and went to confront him, as Tomohiko didn't remember a lot of their shared childhood. The confrontation turned heated, and Naoya accidentally threw Tomohiko off the balcony. Tomohiko suffered a head injury that bled profusely. However, when Naoya tried to show his friends where Tomohiko had fallen, he had vanished, having regained consciousness and returned to the inn.
Yūya Mochizuki later reports that Tomohiko had come to his room, covered in mud and angry. He also admits to telling Tomohiko about the tape. In "Stand by Oneself", Tomohiko begins targeting his classmates, killing Kyōko Kaneki and Aki Matsui by stabbing them from behind. After murdering Aki, he encountered Kōichi, who was infuriated by the senseless deaths. Undisturbed by his actions, Tomohiko attacked Kōichi next, accusing him of being the Extra, but equally furious as he held Kōichi responsible for Yukari's death. Just before he could deal the final blow, however, he was struck from behind by Izumi with an iron rod. Having already encountered Tomohiko's victims, Izumi was furious, declaring he didn't deserve to live, and coldly taunted Tomohiko. However, before she could land the final blow, Tatsuji Chibiki intervened and admonishes Izumi, as classmates shouldn't be killing each other. Furious, Izumi fled with a still unsatisfied antagonism towards Mei Misaki. Mr. Chibiki and Kōichi took Tomohiko's body out to his car, where he'd been gathering the other survivors. Since Mr. Chibiki simply laid out Tomohiko's body and didn't attempt to provide first aid, he realized that Tomohiko was already past help. Naoya was dismayed to learn of Tomohiko's death.
In the manga, none of the events in the 11th and 12th episodes of the anime took place, and he remains alive and sane after being saved by Naoya and Kōichi. In the manga, Naoya had sought help from Kōichi to find Tomohiko's body after he had accidentally pushed him off the cliff. They were glad to find that Tomohiko was still alive and took him back towards the inn to find medical attention for his injuries. They had also found out that he wasn't the "extra person/casualty."
